Would recommend using the version that doesn't expand when you mouse over it, since this image is so massive already. This goes for the other image as well.
It is unknown wether the door itself turned into the RPC-XXX-A instance or wether the door lead to the RPC-XXX-A instance.

To date, no RPC-XXX or RPC-XXX-A leaks have been confirmed, but it is believed that the online myth known as the Backrooms may be related to or based off RPC-XXX, though it cannot be confirmed. Currently, X-Ray-7 are monitoring multiple online forums such as ‘4-Chan’ and ‘Reddit’, but are also keeping a watchful eye on other forms of media.
4-chan doesn't have a dash in the name. Also better to probably just be vague about the media listed here. Seeing reddit and 4chan in an article was a bit of an immersion break. Same goes for the Backrooms meme, which will date itself fairly rapidly.
The following a exploratory logs have been declassified and have been reviewed as safe for viewing.
Remove the 'a' between following and exploratory.
Standard Procedure: Subject is to be tethered and enter RPC-XXX through RPC-XXX-A-α. Subject is to be supplied with a standard Authority sidearm and a weeks worth of rations. Subject is to be informed prior to entry about safety procedures within RPC-XXX. Subject is to be informed to document his surroundings.
This should be in past tense, since this is a thing that happened as opposed to a thing that is intended/supposed to happen.
I didn’t look at it for to muchtoo much as it hurt to look at.
If this is a mistake on the CSD's part, specify it somehow in the text.

Log 9: OH FUCK OH SHIT! I’m infected with something I don’t know what. Actually, I do know what it is. It’s in my head. I followed that lady to a small lake with a red fog floating above it, but it wasn’t actually a fog it was some kind of feeling or emotion. An emotion that was stolen long ago and forgotten by all life. It flowed into me as I breathed it in. Oh god oh fuck Im trying to cut my tether, but the feeling won’t let me.
Log 9 as a whole has mad "Someone's life is in peril and they've decided to spend their limited time writing in a diary" energy.

Remove the space between the dash and the word proper
The entity resembled a short humanoid entity composed of a fragile crystalline material that resembles the shape of a human.
"resembles the shape of a human." is redundant when you've already said its humanoid
A long robotic entity that resembles that if the species Scolopendridae
1. As you said in the footnote, Scolopendridae are a family not a species
2. "that if" should just be removed, as the sentence makes sense without it and its wrong anyway
Luckily, MST Delta-7 and a majority of the research staff were able to flee from OL-Site-035-XXX with major casualties.
I would not describe "major casualties" as lucky.
On December 1st of 2014, a large entity broke through RPC-XXX-A-α managing to collapse Sub-Basement 18 through Sub-Basement 14 and broke through the surface of OL-Site-035 causing minor damage to the Black Lake Technical Institute main campus.
The site not being under Authority control for 6 years feels like a pretty big deal to not mention until the end of the article in an addendum
Descriptions of the entity have been entirely inconsistent with the only consistent attributes being that the entity is quadrupedal and that the entity is large. The entity is believed to mesure between 18 to 32 meters in height.
1. Measure not mesure
2. You call it an entity too much, 4 uses in 2 sentences. Use another pronoun/synonym
Personnel are not allowed to tamper with the tethering equipment if the tether line is seen intersecting into RPC-XXX-A.

RPC-XXX is an extra-dimensional plane located outside of local space-time. RPC-XXX is theorized to have taken entities, concepts, places, groups, and 13 Omega-Black classed RPCs from our reality.
The way "taken" is used here is ambiguous to the point of not making sense. "Taken" means absorbed/incorporated, I assume?
Furthermore, "taken" is incredibly nonspecific and prevents the reader from imagining how this process is supposed to work. How do our plane and RPC-XXX intersect?
I dislike how "Omega-Black" RPCs are specified. This doesn't seem to serve a particular purpose, rather being used to highlight how dangerous/powerful RPC-XXX is.
90% of all encountered RPC-XXX-A instances are unstable and will collapse.
Will -> eventually
All memories and information regarding anything that enters RPC-XXX via RPC-XXX-A will vanish over the course that thing remains within RPC-XXX. Drones sent into RPC-XXX capture nothing but a dark space that seems to go on near infinite.
"All information regarding objects or entities entering RPC-XXX via RPC-XXX-A eventually vanish over the course of its stay. Camera footage of RPC-XXX does not allow perception inside it, as only pitch-black frames have been recorded."
Sapient entities that enter RPC-XXX will describe the space as being many different things as it seems the perception of the space within RPC-XXX is completely unique to each individual entity. Below are accounts of the description of the space within RPC-XXX:
"However, sapient entities entering RPC-XXX are able to describe their sorroundings in a unique manner."
Further description on how this is perceived would help. Is perception nonanomalous? Are inconsistencies present? Does it feel "natural"? Etcetera.
I think listing the name after the example isn't necessary.
Personnel within RPC-XXX are able to interact with things taken within RPC-XXX, though it is ill advised to do so, and return them to our reality
Why?
Entities/Objects taken by RPC-XXX are also capable of exiting RPC-XXX via RPC-XXX-A instance, though certain entities, objects, places, and/or concepts need to posses an sapient organism or entity.
I do not understand what "possess" means here.
Possessed subjects that exit RPC-XXX-A will rapidly transform into the exact entity, object, and/or place. Concepts that exit RPC-XXX through a possess subject do not exhibit the same transforming properties as the other subject classes, though they are designated as being memetic or cognitohazardous RPCs.
Ditto. What exactly is happening here?
Scholars, mystics, and alchemist called this realm “Terra Oblitus”.
The Auctoritas was a Vatican organisation. Why would they have mystics and alchemists?
Currently, X-Ray-7 are monitoring multiple online forums such as ‘4-Chan’ and ‘Reddit’, but are also keeping a watchful eye on other forms of media.
"Currently. X-Ray-7 is monitoring multiple social media sites in search for information relating to RPC-XXX."
While I kinda like your idea, I don't think you do anything interesting with the idea. The explorations feel kinda stilted (with 0909 seeming to know exactly how everything works) and don't make use of the "forgotten" theming very well. It would be very interesting to see a person interact with something completely alien that's supposed to fit into our reality to some degree.
I also think this doesn't work very well, as RPC-XXX doesn't block the acquisition of new memories. Say, if we put a New York replica inside, we would immediately reacquire all lost memories of it, considering its importance.
In general, I think you should try and make what you write collaborate with the central theming and with each other. Everything in the article seems set apart and incoherent as a whole, with each addendum building in a different direction. IE;
Addendum 1 works as a generic "weird place" exploration, working with surrealism. Addendum 2 doesn't seem to do anything with it. Addendum 3, while it seems to build off forgetness as a concept, doesn't seem to interact with RPC-XXX in any way but coming from it, nor do they acknowledge its effects (and the implications of them previously existing) at all. Addendum 4 doesn't do anything with it.
Try and construct the article as a themed whole, not a bunch of disconnected things.
